A record is made
With consent, the centre records what happened, when it happened and who or what can support the account.
The record gains context
Relevant photographs, notes and agreed supporting records are organised so another person can understand what happened.
The Evidence Pack comes together
Related evidence is assembled into a reviewable pack while private raw records remain protected.
MamaDEX prepares the evidence. It does not mark its own work.
Once the Evidence Pack is ready, it can move to a separate assessment process.
